Step 1. Functional assessment
The support measure is assigned as a result of the functional assessment.
The functional assessment system determines the disability status and develops an individual service plan, which indicates what services and support measures the person needs.
Step 2. Medical and social examination
The medical and social examination is carried out to determine the degree of limitation of a person's vital activity, to recognize him as a person with a disability, to establish the status of a "disabled child" and a disability group, to develop and approve an individual rehabilitation program for a person with a disability.
After the examination, the doctor provides a certificate confirming the need for an appropriate assistive device (e.g., wheelchair, hearing aid, prosthesis, etc.).
Step 3. Application to the Unified Social Service
After receiving an individual service plan, the beneficiary or his/her representative must apply to the Unified Social Service (USS) regional center to obtain a state certificate.
What documents are needed:
· application,
· identification document,
· social security card or public services number or certificate of non-receipt of a public services number,
· in case of application by a representative, a written power of attorney from the beneficiary and the representative's passport.
In case of a hearing aid, also a certificate from an otolaryngologist about hearing loss and the need for a hearing aid and audiometry.
In the case of orthoses, in addition to the above documents, those under 18 years of age must also submit a conclusion from a specialized medical organization (regardless of disability), and those over 18 years of age must also submit a conclusion from the Unified Social Service (in case of disability).
Step 4. Obtaining a certificate
After examining the documents submitted by the Unified Social Service, a certificate and a list of service providers are provided.
Step 5. Selecting service providers
The person chooses the organization from which he wants to receive the item and approaches to receive it.
